Aged Care Assessment Team - David Berry Hospital
Overview
Aged Care Assessment Teams (ACATs) help older people and their families and carers establish what type of care will best suit their needs, when they can no longer manager at home without assistance.
What We Do
ACATs provide information on the range of services available in the local community and can help you or your carer to access these services, through information and/or referral.
The staff within each ACAT will vary but may include:
- Nurses
- Social workers
- Occupational therapists
- Physiotherapists.
What to Expect
To undertake the assessment, a member of the local ACAT will visit you at home, in hospital or elsewhere to ask you a series of questions and discuss your options with you. You are welcome to have your usual carer or family member involved in this discussion. There are no fees charged for this assessment.
Who should access this service
Older people (or their families and carers on their behalf) who no longer manager at home without assistance.
How to access this service
Anyone can contact an ACAT – either you, your carer, or a health professional such as your doctor.
