Day Rehabilitation Service

Overview

A Multidisciplinary Day Rehabilitation Program designed to fast track recovery, increase functional independence and allow an earlier discharge from hospital. The Day Rehabilitation Centre has:

  • Rehabilitation Gymnasium
  • Hydrotherapy Pool
  • Kitchen assessment area
  • Group and individual treatment rooms
  • Patient lounge with light refreshments

What we do

We offer a Medical review and Multidisciplinary assessments. We help you to perform everyday tasks safely, and independently

When to access this service

Patients discharged from acute and subacute ISLHD wards requiring MDT intervention for improving independence
Patients with new diagnosis or exacerbation of neurological condition with acute changes in mobility, independence and function

What to expect

Length of program is 4-6 weeks then referral to another outpatient department if appropriate. 1 - 4 days per week with a maximum of 12 sessions over a 4-6 week period. 

Who should access this service

Patients aged 16 years old or older who fit into one of 3 clinical streams (neurological and/or neurodegenerative, orthopaedic, re-conditioning)

How to access this service (referrals or no referral necessary)

You will need a written referral from your Doctor to use this service. All health professionals can refer with relevant doctor completion of the referral form. Your doctor will need to confirm that you require at least two therapies from the MDT team: Physiotherapy, Occupational Therapy, Speech Therapy, Social Work, and Nutrition, Rehabilitation nursing and Multidisciplinary education.

Comfortable clothes and shoes suitable for exercise

Swimmers, thongs and towel (hydrotherapy)

Veterans Affairs Card

Medical reports

Parking and Public Transport

Port Kembla Hospital is located on Cowper Street. Pedestrians can access the hospital from the main entrance on Cowper Street. There is a bus stop located close to the hospital with the following services available - Premier Illawarra 52, 53 and 54.

There is limited parking available for patients, families and carers at the back of the hospital, via Fairfax Road.

Advice for disabled in getting to the service

There are disabled parking spots in the car park. The drop off zone at the front door of the hospital can be accessed via the entrance on Cowper Street.
