What the service is

Community Home-based service for adults over 18 years.  

What we do

The Domiciliary Occupational Therapist works with clients with a broad range of medical diagnoses, including but not limited to neurological, orthopaedic, frail aged and spinal conditions.  This may involve Falls prevention management, Pressure Injury management, Home modifications – minor and complex, Equipment prescription – wheelchairs, pressure relieving devices, hospital beds, bathroom aids/appliances such as shower chairs/over toilet aids. 

What to expect

Comprehensive assessment and management of your condition. Prescription of appropriate devices/modifications to assist you in continuing to live independently and safely at home.

How to access this service

Referrals can be made by a nominated treating doctor, specialist, allied health professional or registered nurse.  Referrals are required in writing, and can be faxed, mailed or hand delivered to the Port Kembla Hospital Occupational Therapy Department.

If over 65 years and older or 50 years and older if you are an Aboriginal or Torres Strait Islander person then a My Aged Care referral is required. Referrals must be made through the My Aged Care website or by calling My Aged care on 1800 200 422.

How to get there, including by public transport

Generally this is a Community home based service and as such appointments will be made to meet in your home. However should circumstances warrant a visit to the hospital , then access to the facility can be gained via Cowper Street or Fairfax Avenue

Head of Discipline - Occupational Therapy

Kate Andersen is the Head of Discipline for Occupational Therapy and Diversional Therapy. She also provides leadership to the equipment loan pools across the District. Kate brings a vast variety of clinical experience to the role, with almost 20 years within the local area.

The team of Occupational Therapists work in partnership with clients to maximise independence in their activities of everyday life, enhance safety and optimise their quality of life. Services are provided in hospitals, community health centres and in the home environment.
