Aboriginal Chronic Care Unit - Nowra
Overview
The Aboriginal Chronic Care Unit (ACCU) is an Aboriginal and Torres Strait Islander chronic disease management service within the Illawarra Shoalhaven Local Health District.
The ACCU aims to improve access to health services for Aboriginal and Torres Strait Islander people living with chronic disease. This is achieved through close partnerships with internal and external service providers and clinicians to develop strong referral networks and health management pathways. The ACCU works to develop the skills of Aboriginal Health Workers (AHW) and Aboriginal Health Practitioners (AHP) to meet the needs of the Aboriginal community.

- Shoalhaven Aunty Jean’s Chronic Disease Program
- Ulladulla Aunty Jean’s Chronic Disease Program
- Facilitation of outreach services to Jervis Bay Territory (Wreck Bay community and Jervis Bay Village) and the Orient Point (Jerrinja) community
- Post-48 hour Follow-up service
Who should access this service
Aboriginal and Torres Strait Islander people in the Illawarra and Shoalhaven region aged 15 years and above living with a chronic condition, or those interested in living a healthy lifestyle.
