Aboriginal Chronic Care Unit - Port Kembla

Overview

The Aboriginal Chronic Care Unit (ACCU) is an Aboriginal and Torres Strait Islander chronic disease management service within the Illawarra Shoalhaven Local Health District.

The ACCU aims to improve access to health services for Aboriginal and Torres Strait Islander people living with chronic disease. This is achieved through close partnerships with internal and external service providers and clinicians to develop strong referral networks and health management pathways. The ACCU works to develop the skills of Aboriginal Health Workers (AHW) and Aboriginal Health Practitioners (AHP) to meet the needs of the Aboriginal community.

  • Illawarra Aunty Jean’s Chronic Disease Program
  • Illawarra Koori Aqua Aerobics Program
  • Post-48 hour Follow-up service

Who should access this service?

Aboriginal and Torres Strait Islander people in the Illawarra and Shoalhaven region aged 15 years and above living with a chronic condition, or those interested in living a healthy lifestyle.
