Homelessness Mental Health Program
Overview
The Homelessness Mental Health Program works with referring and supporting organisations to help establish new or maintain existing accommodation for clients with a mental health diagnosis, who are at risk of homelessness, have a tenancy at risk, history of homelessness, or are transitioning to secure accommodation.
We offer assistance by either co-case management or brokerage, to achieve better housing and mental health outcomes. Brokerage applications are presented to a committee for consideration and approval.

Co-Case Management
We work with referrers to improve care coordination through education, information, support and advocacy to improve an individual's mental health and housing outcomes alongside monitoring and evaluating service provision. We work as a point of consultation and in partnership with organisations in the community.
Brokerage
Funding is available to purchase items and/or services that will help sustain current accommodation or help reallocate to more appropriate accommodation.
Referrals
Access to this program is by referral from a supporting community agencies or health service. Note: Referral to be completed by referrer (not client).
Referrers - NGOs (Mental Health, Housing, Youth, Family, Legal, Tenancy Advocacy, Aboriginal Support and Medical Services), Health (Mental Health, Social Work, Inpatient and Community services)and Government Departments (DCJ - FACS and Housing, Community Corrections).
