Community Health Nurses deliver nursing care to people at community health centres and at home in the Illawarra and Shoalhaven.

We provide specialist nursing services with the goal of empowering clients through education and support to enable them to maximise self- management of their condition.

We will work with you, your carers and other services to provide the care you need.

Our aim is to help you improve or maintain your health and wellbeing so that you can continue to live independently.

We provided nursing care for the following conditions:

• Post-Acute (hospital)
• Wound Care (acute and complex)
• Palliative Care
• Catheter Care and Education

Specialist Nursing Services are also offered in the community including:

• Asthma Service
• Continence Service
• Wound Care Specialist Service  
• Stomal Therapy

Referrals for all Community Nursing Services are made through the Access and Referral Centre.
